You need a X1541 or XE1541 cable, a spare disc drive and a parallel port on your PC, plus some transfer software such as Star Commander. Goto here for more info:

Where you can get the software (they wrote SC) and either be shown how to build the cable or buy one from them (about $10).
You connect the disc drive to the PC using the cable. Then you use the software to transfer the binary image from the PC over onto a real 5.25" disc in the disc drive. Works great for me! It will also work for making .d64 files from real discs too.

Yeap, I've transferred a few .d64 images to ral C64 disks, works great, and it's brilliant to be able to play them on a real C64 as well (although I haven't done that in a couple of years..)
